Cost estimates are based on the information available at the time they are run, typically up to one week prior to your appointment. They may not reflect recent services received from outside providers or facilities. Estimates are calculated using the contracted rates between your insurance and the clinic and may apply toward your deductible, coinsurance, and/or out-of-pocket maximum depending on your insurance coverage and the type of appointment. Estimates are not a guarantee of final charges.

If your payment results in an overage, a refund will be issued within 45 days once a credit is identified on your account.

If your insurance ultimately pays less than the original estimate, you will be responsible for any remaining balance after all time-of-service payments and insurance payments have been applied.
